You, _______________________, have been found guilty of the sin of _________________________, a sin deserving of the extreme penalty of excommunication, yet you continue obstinate and give no evidence of repentance, although efforts have been made to restore you. Now, therefore, this Court, constituted in the Name of the Lord Jesus Christ, and acting under His authority, hereby excommunicates you, removing you from the membership of the Church, outside of which there is no ordinary possibility of salvation. May God have mercy upon you by awakening you to repentance and bringing you to righteousness, so that you may be among the redeemed in the Day of the Lord Jesus.
