Whereas you, __________________, have been found guilty of the sin of ________________________, this Court, in the Name of the Lord Jesus Christ, sadly and solemnly suspends you from the privileges of church membership, including participation in the sacraments, until you have given satisfactory evidence of true repentance, and have been restored to good standing by this Court.
